The Best Shot For Avoiding Flu,
For four-year-old Rahul Arya, the onset of winter is a painful time. An asthma patient, he lives in constant fear of falling prey to influenza, a common ailment during winter. Even a slight cold and fever makes his breathing difficult and he has to be on a nebuliser for days after that. An attack of influenza can make things more complicated. The only hope that he has is a shot of the flu vaccine. Sadly, his parents have not heard about it.
``Influenza is a serious threat in the country, especially among people who have a lung disease. The flu vaccine can save people from falling victim to serious complications but unfortunately, the awareness is very low,'' says Dr Sanjay Lalwani, HOD, paediatrics, Bharati Vidyapeeth University Medical College, Pune. It's particularly tough for asthmatic children. ``At our nursery care centre, we get at least 40-50 admissions of children suffering from influenza during the flu months, which are from October to March. Things wouldn't be so bad if they had been given the vaccine,'' he adds. The flu vaccine is the most effective prevention for high risk patients. Its given before the onset of flu months. But in a tropical country like India, where the virus circulates round the year and not necessarily during winter, it can be given any time. Children between the ages 6 months to five years, and adults above 65 years, should ideally take the vaccine. But it's essential for those suffering from diabetes, pulmonary or cardiovascular illness, renal dysfunction and even pregnant women --i.e. all those in the highrisk group. Unlike other vaccines, this one needs to be given annually because the virus keeps changing its surface antigens and accordingly, the vaccine's composition needs to be changed. The World Health Organisation, through its Influenza Surveillance Network, monitors the
change and accordingly decides on the strain to be used in the vaccine every year. Unfortunately, on the one hand, awareness about the vaccine is pathetically low in India and, on the other, the government has not made it mandatory. ``The vaccine is underutilised in our country,'' says Dr Nitin Shah, consultant paediatrician, Hinduja National Hospital, Mumbai. Most people don't know that it gives nearly 70-90% protection from influenza and complications arising from the infection, while being completely safe and without side-effects. The lack of knowledge about the potential risk of influenza is also pathetic. Most people dismiss it as a severe form of common cold, little knowing that if left untreated in highrisk patients, it can be fatal. In the US alone, influenza kills nearly 36,000 people every year. And this is despite the fact that awareness about the vaccine is much higher in the West. No such statistics are available for India but experts believe that the figures could be even higher. ``It's really sad that no influenza infections are reported in hospitals. Even general practitioners are not trained enough to differentiate between influenza and common cold,'' rues Prof A K Prasad, chairman, Influenza Foundation of India. While the highest infection rates are reported among children between the ages 5-9, it's those below two years who are at high risk. Globally, the attack rate in children is reported at 20-30% while it's 5-10% in adults. For the high-risk group, a combination of factors -- pollution, cigarette smoke, primary flu and overcrowding can lead to an attack of pneumonia and, in many cases, death. ``Unlike the West, we still don't have a concept of preventing it, we only look at cures,'' says Dr Randeep Guleria, senior consultant, department of medicine, AIIMS. Prasad is optimistic that the scenario will improve. ``Just as awareness of the hepatitis B vaccine spread and its cost came down from Rs 1,000 to Rs 25, I am sure the same will hold true for influenza vaccine (which costs Rs 540 at present),'' he says. A heart patient, he regularly takes the influenza vaccine. At AIIMS, Dr Guleria has seen the number of elderly getting vaccinated go up by nearly 20% in the last two-three years. From: TOI, Jan-06-08
